The main inspiration for PAIN's exclusive PlayStation 3 game was the Jackass TV show. The action of this position takes place in a big city, where the protagonists participate in various situations, commonly referred to in society as extreme.

Modes include PAINdemonium, Fun With Explosives, Spank the Monkey, Mime Toss, HORSE, and many more.
